ST. PETER — A state agency said an addiction treatment center in St. Peter was neglectful in the death of a patient.

The Minnesota Department of Human Services found the Community Addiction Recovery Enterprise facility failed to take basic precautions that could have prevented the suicide of 24-year-old Ater Manyang.

A state investigation showed that while Manyang’s four previous suicide attempts were documented in his medical records, staff at the treatment facility were largely unaware of them and failed to increase supervision of the patient.

According to a press release, the former basketball star at Century High School in Rochester had been civilly committed last March due to mental illness and chemical dependency.

State Human Services Commissioner Emily Johnson Piper said her agency has made changes to improve the assessment of patients’ suicide risks and how it’s communicated to staff.
